Soccer tryout evaluation form with ten soccer skills rated one to five, notes and a decision line

This free soccer tryout evaluation form lets each evaluator rate a player on 10 soccer skills from 1 to 5, write notes on strengths and areas to develop, total the score out of 50 and record a decision. There are two player cards on every page, so a full tryout fits in a short stack of paper.

What’s on the sheet

  • The tryout number first, in a box — match it to the number on the player — then player name, school, birth year or grade and position
  • Ten skills rated 1–5: first touch, passing, dribbling and 1v1, shooting and finishing, defending and tackling, positioning off the ball, speed and agility, stamina, soccer IQ, coachability and effort
  • A notes column per skill, a total out of 50 and a decision line (team, development group or call-back) with the evaluator's name
  • Ruled space for each player's strengths, areas to develop and additional notes

How to fill it in

  1. Put a tryout number on every player (pinned bib or tape) and write the same number in the box at the top of their card, so evaluators rate by number, not by name.
  2. Assign each evaluator a few skills or a group of players rather than everything at once.
  3. Rate what you see on the day using the scale on the sheet: 1 needs significant work, 3 at level, 5 standout.
  4. Total the scores, but compare totals only within the same age group, and use the notes to break ties.
  5. Record the decision and the evaluator's name so you can explain choices to parents later.

Questions people ask

How do you make soccer tryouts fair?

Use numbers instead of names, have at least two evaluators rate each player, agree on what 1–5 means before tryouts start, and keep the forms so decisions can be explained.

What should I look for at soccer tryouts?

First touch and decisions under pressure — they show up in small-sided games far more clearly than in drills. Watch what players do without the ball, too: positioning and work rate.

How do I evaluate goalkeepers?

Most of the field-player skills on this form don't apply to keepers. Use the notes column for handling, positioning, distribution and decision-making, or rate keepers on a separate session.

How many evaluators do you need at soccer tryouts?

At least two, so every player is rated by more than one person. With three, you can ignore the score that sits furthest from the other two.
