Soccer Lineup and Substitution Sheet (Free PDF)
By the CourtClok team · Updated October 2026

This free soccer lineup sheet puts your starting 11 on a pitch diagram and gives you a minutes-played grid for up to 18 players, split into 10-minute blocks — so you can plan substitutions before the game and make sure every player gets fair time on the field.
Free to print and share. No sign-up.
What’s on the sheet
- Team, opponent, date and formation
- A pitch diagram with 11 position boxes (laid out as a 4-3-3, but every box is blank so you can write any formation)
- A minutes-played grid for 18 players in 10-minute blocks, with a total column
How to fill it in
- Write your formation at the top and each starter's name or number in a position box. Ignore the position labels if you play a different shape.
- List every player in the minutes grid before the game.
- Plan substitutions in advance: tick the blocks each player is meant to play so everyone's planned total is visible at a glance.
- During the game, tick (or correct) each block a player actually spent on the field, then add up the totals afterwards.
- Adjust the blocks to your game length — for shorter youth games, use only the first few columns.
Questions people ask
How do I make sure every player gets fair playing time?
Plan it before kick-off. Tick the blocks you intend each player to play, check the totals are close, then follow the plan. Writing it down stops the strongest players quietly playing the whole game.
What formation is best for youth soccer?
There isn't one right answer — it depends on the number of players per side and your players. The sheet's boxes are blank so you can write in whatever formation you use.
Can I use this sheet for 7v7 or 9v9?
Yes. Use as many position boxes as you need and leave the rest empty; the minutes grid works for any number of players up to 18.
