It records both rosters (starters, sub-in and sub-out minutes, goals, yellow and red cards), every goal with the minute, scorer and assist, all cautions and send-offs, and the score by half, extra time and penalties.

What’s on the sheet

  • Match details: league, date, field, kick-off time, both teams, referee and age group
  • Home and away rosters of 18 players: starter tick, sub-in and sub-out minute, goals, yellow and red cards
  • A goal log: minute, team, scorer's number and assist
  • Cautions and send-offs: minute, team, player, card and reason
  • Score by first half, second half, extra time and penalties, plus referee and coach signatures

How to fill it in

  1. Before kick-off, fill in both rosters with jersey numbers and tick the starting players.
  2. When a goal is scored, write the minute, the team, the scorer's number and the assist (if there was one) in the goal log, and add it to the player's Goals column.
  3. For each substitution, write the minute in the incoming player's Sub in box and the outgoing player's Sub out box.
  4. Log every yellow or red card with the minute, player and reason, and mark the player's Y or R box.
  5. At half-time and full-time, fill in the score boxes. Add extra time and penalties only if the match needs them, then collect signatures.

What goes on a soccer game sheet?

The teams and rosters, who started, every substitution with its minute, every goal with scorer and assist, every card with the reason, and the score by half. That's what a referee or league needs to confirm a result.

How do you record a substitution?

Write the minute of the change in the incoming player's Sub in box and the same minute in the outgoing player's Sub out box. If your league limits re-entry or the number of substitutions, check the sheet against that limit at half-time.

How long is a youth soccer game?

It depends on the age group and league — younger groups play much shorter halves than high school. The sheet records minutes, so it works for any game length.

Who should keep the game sheet?

Usually the referee or a designated team official. If a league doesn't provide one, it's worth having one parent per team keep it so both sides agree on the result.
