This free basketball tryout evaluation form lets each evaluator rate a player on 10 skills from 1 to 5, add notes, total the score out of 50 and record a decision. There are two player cards on every page, so a full tryout fits in a short stack of paper.

What’s on the sheet

  • Player name, tryout number, birth year or grade and position
  • Ten skills rated 1–5: ball handling, shooting, finishing, passing, on-ball defense, help defense, rebounding, speed and conditioning, basketball IQ, coachability and effort
  • A notes column, a total out of 50 and a decision line (team, development group or call-back) with the evaluator's name

How to fill it in

  1. Give every player a tryout number (pinned bib or tape) so evaluators can rate without knowing names.
  2. Assign each evaluator a few skills or a group of players rather than everything at once.
  3. Rate what you see on the day using the scale on the sheet: 1 needs significant work, 3 at level, 5 standout.
  4. Total the scores, but compare totals only within the same age group, and use the notes to break ties.
  5. Record the decision and the evaluator's name so you can explain choices to parents later.

Questions people ask

What should I look for at basketball tryouts?

Skills you can't teach quickly — effort, coachability, decision-making and how a player competes — matter as much as shooting. The form gives each its own row so they don't get lost behind the obvious skills.

How do you make tryouts fair?

Use numbers instead of names, have at least two evaluators rate each player, define the 1–5 scale before tryouts start, and keep the forms so decisions can be explained.

Can I use this form for volleyball or other sports?

The layout works for any sport, but the skills listed are basketball-specific. For volleyball you'd swap in serving, passing, setting, attacking and blocking.
