This is a free, one-page basketball score sheet you can print and use at any youth, school or rec game. It tracks both teams on a single landscape page: rosters, personal fouls (1–5), team fouls and timeouts by period, points by quarter and overtime, and a running score up to 160.

- Game details: league, date, court, game number, both teams, referees and scorekeeper

How to fill it in
- Before tip-off, fill in the game details and both rosters with jersey numbers. Ask each coach to confirm the numbers.
- When a basket is scored, cross off the next number in that team's running-score column (A or B) and write the scorer's jersey number beside it. Use a single slash for a free throw and an X for a field goal if you want to tell them apart later.
- Add the points to that player's box for the current quarter.
- For every personal foul, mark the player's next foul box and tick one team-foul box for the quarter. Tell the referee when a player reaches their fifth foul.
- Tick a timeout box each time a coach calls one.
- At the end of each quarter, copy both teams' running scores into the summary table. At full time, total each player's points, check they add up to the running score, and sign.
Questions people ask
How do you keep score in a basketball scorebook?
Use the running score as the official record: every time a team scores, cross off the next number(s) in its column and note the scorer's jersey number. Each player's quarter box and the team total must add up to the running score at the end. If they don't, the running score is usually right and a player box is missing points.
How many fouls before a player fouls out?
Five personal fouls in high school (NFHS) and FIBA rules; six in the NBA. Many youth leagues follow five, but check your league's rules. The sheet has five boxes per player for that reason.
When does a team reach the bonus?
It depends on the rule set. Under current NFHS high-school rules, the fifth team foul in a quarter puts the opponent in the bonus (two free throws) and team fouls reset each quarter; FIBA also awards free throws from the fifth team foul in a quarter. Older high-school and many rec leagues still use halves. Use the per-quarter team-foul boxes either way and confirm the rule with the referee before the game.
Is there a basketball score sheet for 15 players?
This sheet fits 12 players per team so both teams and the running score fit on one page. For a bigger roster, print a second copy for the extra players or use a stat sheet per team.
What's the difference between a score sheet and a stat sheet?
A score sheet is the official record of the game: score, fouls and timeouts for both teams. A stat sheet tracks individual performance (shots made and attempted, rebounds, assists, turnovers) and is usually kept per team by a coach or parent.
